I’ve known Dr. Bose for nearly 10 years as she has cared for my daughter. Today, l was blessed to have her help my 83 year old mother. You will not find a brighter, more compassionate doctor!   • Surgical procedures, such as joint replacement
  • Controlled substance prescriptions, i.e., narcotics/opioids (e.g., Tramadol, Tylenol #3, Tylenol #4, Percocet, Vicodin), certain muscle relaxants (e.g., carisoprodol, tizanidine, methocarbamol, metaxolone), anxiety medications (e.g., alprazolam, lorazepam), sleep medications (e.g., zolpidem), certain "nerve" medications (e.g., pregabalin, high dose gabapentin) and any other medications which may be beyond the scope of our practice
  • After hours and weekend coverage, so please be sure to ask for your medication refills during business hours
  • Evaluation and paperwork for workman's compensation, short-term or long-term disability (we, CAN, however, assist with FMLA forms)

    • Medicare Non-PAR: We are non-participating with Medicare. We can treat Medicare members. The patient pays for the office visit up front. The office submits the claim for the member to Medicare, and Medicare reimburses 80% of the allowable. If the member has a secondary insurance plan, the secondary insurance plan reimburses the remaining 20%.

    **HMO plans need a referral authorization from the PCP**

    **We do not accept Medicaid, Ambetter or Tricare(as a primary insurance) currently**
